As an alternative to residential nursing facilities, InnovAge LIFE (Living Independence for the Elderly) helps seniors keep living at home. Our neighborhood care center provides a wide range of healthcare and social services and benefits all in one place.

To participate in InnovAge LIFE you must be:

  • At least 55 years old
  • Able to live safely at home and in the community with support
  • Eligible for nursing facility level care
  • Living in an InnovAge PACE service area (call to confirm that your address is included)

What is InnovAge PACE?

InnovAge PACE (Program of All-inclusive Care for the Elderly) is an alternative to nursing facilities. It provides customized healthcare and social engagement to older adults to help them stay independent and in their homes. PACE participants receive most of their healthcare and social support at a PACE center in their community. Each person in the program has a team of medical experts and a personal care plan that is focused on providing a personalized PACE experience. InnovAge is one of more than 100 PACE providers across the U.S.

Who can join InnovAge PACE?

To participate in PACE, you must be

  • At least 55 years old;
  • Eligible for nursing facility level of care; and
  • Living in an InnovAge PACE service area.

How much does it cost?

*If you live in or move to supportive housing (assisted living or nursing home), there is a co-pay to that facility. Also, some states require a share of cost fee if your income exceeds the Medicaid threshold.

Does Medicaid or Medicare cover InnovAge PACE services?

Does anyone live at an InnovAge PACE center?

No one lives at an InnovAge facility. While InnovAge provides full, wrap-around, coordinated healthcare, the seniors enrolled in PACE live in their own homes or in supportive housing based on their individual needs.
