What to Expect from PACE

InnovAge is a healthcare provider for senior citizens. InnovAge runs a program called PACE, a Program of All-Inclusive Care for the Elderly. When a senior is an InnovAge participant, they continue to live at home, and they come to an InnovAge PACE center to receive all of their medical care - and so many other services that allow seniors to live happily and healthily at home.

PACE is an all-inclusive healthcare program for seniors that provides all healthcare at a PACE center. 

All PACE participants have a team of 11 healthcare professionals, all working together to create personalized healthcare plans around the individual goals and needs of each and every participant. With specialized focus on working with seniors, our team knows the ins and outs of senior healthcare. The team makes it easy for participants to focus on living more happily and healthily, because every single need is taken care of, including:

All InnovAge participants must meet the following criteria:

  • Age must be 55+ years old
  • Able to live safely at home or in a community with some support
  • Need help with daily activities or tasks - things like remembering to take medication, help with buttoning shirts or tying shoes, or even rides to medical appointments
  • Live in an InnovAge service area (don't worry - we'll check for you!)

What is InnovAge PACE?

InnovAge PACE (Program of All-inclusive Care for the Elderly) is an alternative to nursing facilities. It provides customized healthcare and social engagement to older adults to help them stay independent and in their homes. PACE participants receive most of their healthcare and social support at a PACE center in their community. Each person in the program has a team of medical experts and a personal care plan that is focused on providing a personalized PACE experience. InnovAge is one of more than 100 PACE providers across the U.S.

Who can join InnovAge PACE?

To participate in PACE, you must be

  • At least 55 years old;
  • Eligible for nursing facility level of care; and
  • Living in an InnovAge PACE service area.

How much does it cost?

Does anyone live at an InnovAge PACE center?

No one lives at an InnovAge facility. While InnovAge provides full, wrap-around, coordinated healthcare, the seniors enrolled in PACE live in their own homes or in supportive housing based on their individual needs.
