InnovAge Again Recognized for Patient-Centered Care

InnovAge's Colorado PACE centers have been recertified as Patient-Centered Medical Homes for the third time. This designation reinforces InnovAge's commitment to delivering quality, patient-centered care.
"Achieving the recognition for the third time demonstrates our staff's continued commitment to high quality, individualized care," says Gina DeBlassie, chief operating officer for the central region. "Our participants benefit through the experience in the delivery of services and the patient empowerment at the center of the PCMH recognition."

Colorado PACE centers were first certified by the National Committee for Quality Assurance (NCQA) in 2015. “We are the only PACE program nationally to have achieved and maintained this certification,” says Dr. Trish Litvak, who is leading InnovAge's PCMH efforts. "This is a special distinction as only 20% of physician practices nationally achieve this honor."
InnovAge is voluntarily and independently pursuing this certification for all of its PACE and LIFE centers in all of its existing states - beyond state and federal requirements. "It reflects our ongoing focus on standardizing policies and procedures, comprehensive care coordination, and continuous quality improvement," explains Dr. Litvak.

Achieving PCMH recognition demonstrates InnovAge's commitment to six key standards:

  2. Knowing and managing patients
  3. Patient-centered access and continuity
  4. Care management and support
  5. Care coordination and care transitions
  6. Performance measurement and quality improvement
