InnovAge Recognized for Placing Patients at the Forefront of Care

A new certification for InnovAge recognizes the organization’s commitment to delivering quality, patient-centered care. The National Committee for Quality Assurance (NCQA) recently certified the PACE centers in Colorado as Patient-Centered Medical Homes (PCMH).

Dr. Trish Litvak, senior medical director for care transformation is leading the InnovAge effort to expand PCMH recognition to all PACE and LIFE centers. “Standardization of policies and procedures and continuous quality improvement are key to the process,” says Dr. Litvak.

Below are some of the areas that are measured via the PCMH certification process.
  • Demonstrating continuity of care as a team and providing patient-centric care.
  • Monitoring and analyzing information about participants and communities to deliver evidence-based care to support their needs and in a culturally and linguistically appropriate way.
  • Providing 24/7 access to clinical advice and care.
  • Care Management and Support-we identify the needs at the individual and population level to plan and coordinate care. We use care and case management to identify individuals with chronic illness who may use services more who could be managed more effectively within our centers.
  • Tracking tests, referrals, and transitions (such as leaving the hospital for home or respite) to ensure patient safety and communication with specialists and other providers.
  • Measuring outcomes and identifying and working on areas of improvement.

NCQA President Margaret E. O’Kane says, “Recognition shows that InnovAge Colorado PACE has the tools, systems and resources to provide its patients with the right care, at the right time.”

The six certified centers are the Aurora PACE center, Denver PACE center, Lakewood PACE center, Thornton PACE center, Loveland PACE center, and Pueblo PACE center.
