Regional Executive Director - West

Taking care of people and listening to their stories gives me immense satisfaction and I am happiest when I can help someone along their healthcare path. I feel privileged to be a part of the PACE team at InnovAge and help our seniors live the life they deserve.

Jeanette Williams joined the InnovAge team in 2019. She has more than 20 years of experience in healthcare leadership. Her experience encompasses running long-term acute care, acute rehab hospitals, home healthcare, and skilled nursing. She holds a master’s degree in business administration from University of Phoenix.

Center Director

I feel honored to play a role at helping PACE participants to achieve evolving health and quality of life goals. It is a privilege to work with dedicated teams who share the commitment to sustain the independence of seniors and to improve access locally/nationally to premiere senior care services.  

Patricia Sandoval has over 16 years of diversified healthcare leadership experience. She has supported FQHCs to improve operational efficiency, expand access in rural communities and to achieve high quality care standards with Joint Commission and Patient Centered Medical Home Recognition. Between 2006-2012, Patricia provided administrative leadership for Senior Care Management programs at AltaMed Health Services. More recently, Patricia held roles as the Executive Director for Fresno PACE (serving more than 800 participants) and as a Center Manager for On Lok PACE in San Jose. She also served as a CalPACE Board Member between 2017-2020 and as a consultant for the NPA PACE 2.0 National Learning Collaborative that would help participating PACE organizations to increase access to and utilization of PACE services.

Patricia holds a Master’s degree in Public Administration from the University of San Francisco and a Bachelor’s Degree in Business Administration from the University of Phoenix.

Dr. Siraj is board certified in family and geriatric medicine. She earned her doctor of medicine from Kempegowda Institute of Medical Science in India. Dr. Siraj completed her residency at the University of Arkansas for Medical Science and her fellowship at the Reynolds Institute on Aging. Prior to joining InnovAge, Dr. Siraj worked in private practice, served as a mentor to medical students and residents and was a medical director for hospice.

Dr. Siraj is a physician with Integrated Long Term Care Medical Group, which provides California-licensed physicians and nurse practitioners to serve participants of InnovAge California PACE (ICAP).

Dr. Siraj is married and has two children. She enjoys swimming, reading and traveling.

Nurse Practitioner

I want to provide PACE participants with the best care. The same care I want to have when I am their age.

Crystal earned her nursing degrees at Azusa Pacific University. She is board certified as a family nurse practitioner by the American Association of Nurse Practitioners. While she was in college, she worked as a volunteer at the Veterans Administration (VA) Loma Linda Healthcare System in the emergency department and intensive care unit.

Crystal is a home tutor and has been teaching Mandarin for more than 10 years. She also speaks Chinese. Crystal lives in Redlands, CA and has a teenage son. She enjoys hiking, jogging and spending time with family.

Nurse Practitioner

Life can be full of unexpected twists and turns and it’s not always easy. Through faith and family support, I can always find joy in what I’m doing - especially when I’m serving others. I try to approach each participant with compassion and empathy. I hope I can help them live life on their terms – aging well and enjoying a good quality of life.

Clinton earned his Master of Science, Family Nurse Practitioner, from Graceland University in Iowa and a Bachelor of Science, Nursing, from Grand Canyon University in Arizona. He is board certified as a Family Nurse Practitioner by the American Nurses Credentialing Center (ACNN). He completed residencies in internal medicine, pediatrics, and family practice.
 
His medical career began as an emergency medicine nurse at a trauma center hospital in Las Vegas, Nevada. He later moved to California and worked as an emergency department nurse at Riverside Community Hospital and at Loma Linda University Hospital. Clinton also worked as a nurse with a pandemic relief project serving the Los Angeles County homeless population. In addition to his traditional duties as a nurse, Clinton has always sought ways to support new nurses through formal preceptor programs and mentoring opportunities.
 
Clinton is passionate about his family, which includes his wife, three children and two Chihuahuas. They enjoy spending time at the beach, boating, visiting zoos, aquariums and Disneyland. His personal hobbies include fitness, fishing and recreational target shooting.

InnovAge PACE is very important for seniors. I enjoy providing them with complete dental care they may not have had access to in the past.

Dr. Huigens earned his Doctor of Dental Surgery Degree (DDS) from the University of Oklahoma Health Sciences Center, College of Dentistry where he graduated with honors. He also earned a Physician Associate Degree from the University of Oklahoma College of Medicine and a bachelor degree in biology from the University of LaVerne in CA.

Upon graduation, we was in private dental practice in La Verne, CA. Dr. Huigens has a special interests in dental implants and dental appliances to treat sleep apnea. He also volunteered as a dentist at a low-income children’s dental clinic.

Dr. Huigens has always been interested in healthcare and nearly every job he’s held has been in the medical profession. As a college student, he worked as a paramedic and as a physician associate at an orthopedic surgery practice.

In his spare time, Dr. Huigens enjoys art, music, do-it-yourself projects, working on his car and spending time with his son.

Nurse Practitioner

Every opportunity in life provides us with an opportunity to express our faith and purpose. My role at InnovAge is a way to purposefully step forward with love, kindness and respect to help seniors achieve better health.

Justina earned a master’s degree in nursing, family nurse practitioner from the University of Phoenix. She also has a bachelor’s degree in law from the University of London and  a PhD in International Law from Northwestern California University School of Law. Her clinical interests include health promotion, prevention, home health, hospice and palliative care.

Prior to joining InnovAge, she served as director of patient care services at Trinity Hospice Inc in California. Justina has additional work experience in geriatric care, home health, palliative care, hospice services and legal nurse consulting.

Justina is very involved in her community. Some of the organizations she works with include Women of Grace Ministries, Living Faith Anglican Communion, Nigerian American Political Action Committee and All Nigerian Nationals in the Diaspora.

Her life experiences include a mix of many cultures. Justina left Nigeria as a teenager and moved to London to complete her general education and an undergraduate degree. She moved back to Nigeria and practiced corporate law for a couple of years. She then moved to Florida and later to California. She and her husband have two children. Justina’s faith is the cornerstone of her life. She also likes to read, cook and entertain others in her spare time.

I am happy to be part of PACE and the unique team approach to care. I have learned from participants to always listen first. PACE is an amazing program for the elderly.

Kate is a board certified Family Nurse Practitioner. She earned her nursing degrees at Loma Linda University School of Nursing. She completed her residency at Loma Linda University and the Department of Veterans Affairs, Loma Linda.

Prior to becoming a Nurse Practitioner, she worked as a forensic nurse with the State of California. Her past work experience includes skilled nursing facilities, hospice care, psychiatric nursing and wound care.

Kate was born in West Africa, Ghana and speaks Twi. In her spare time she enjoys being at home with her family. They like to watch movies. As a California resident for more than 20 years, she enjoys going on long rides for adventure.

My goal is to provide compassionate, competent and holistic care to each of our participants. I am fortunate to work at InnovAge.

Keith earned his Bachelor’s and Master’s Degrees in Nursing at California Baptist University in Riverside, CA. He is board certified as a Family Nurse Practitioner by the American Association of Nurse Practitioners (AANP).

His experience includes nursing and nurse practitioner positions in many clinical settings, including; San Bernardino Community Hospital, Loma Linda Department of Veteran’s Affairs, Riverside County Regional Medical Center, Human Resource Network (traveling nurse), Arrowhead Regional Medical Center, Golden Shore Medical Center, and Unicare Community Health Center. Keith also serves in teaching positions at College of the Desert in Palm Springs, CA and Chaffey Community College in Rancho Cucamonga, CA.

Keith enjoys spending time with his family, especially his nieces and brothers. He has two dogs. Keith’s hobbies include weight training, biking, hiking and boxing. He likes to attend concerts, theater and fine arts. He also enjoys music, writing and reading. Keith is active in his community on half of social issues important to him.

Primary Care Doctor

I see medicine not just as a science, but as an art, a calling, and a ministry. My desire is to offer my patients personalized and comprehensive healthcare that fits their life goals and aspirations. I believe we can make that a reality with PACE.

Dr. Florian earned her doctor of medicine degree at Loma Linda University in California and a bachelor's degree in biology at Antillean Adventist University in Mayaguez, Puerto Rico. She is board certified in family medicine and geriatric medicine. Dr. Florian completed a family medicine residency at Bella Vista Hospital in Mayaguez, Puerto Rico and a fellowship in geriatric medicine at The Christ Hospital/University of Cincinnati in Ohio.

Dr. Florian was born in Lima, Peru and moved to the United States as a young child. She enjoys learning about new cultures and cooking foods from around the world. She also enjoys baking healthy, vegan treats and is writing a vegan cookbook. Her other hobbies include classical and baroque music, hiking, and meditating. She is fluent in Spanish.

Primary Care Doctor

I believe in PACE and its multidisciplinary team approach to geriatric care that allows participants to focus on what is important in their lives.

 

Dr. Tang earned his medical degree from the A.T. Still University School of Osteopathic Medicine in Arizona. He also completed a family medicine residency at CarePoint Christ Hospital in New Jersey and a geriatric medicine fellowship at the University of California in Irvine, California. Dr. Tang is board certified in Family Medicine.

Dr. Tang is fluent in Mandarin. He enjoys traveling, biking, hiking and exploring new areas.

I love the all-inclusive care provided here at PACE. I always strive to provide evidence-based care and work together with participants to overcome obstacles.

Matilda is earned her Doctor of Nursing Practice at South University of Savannah, in Savannah, GA. She completed the Adult Primary Care Nurse Practitioner Residency Program, at South University of Savannah in Savannah, GA.  Matilda is board certified as a nurse practitioner. She enjoys learning different medical procedures and is particularly interested in pain management.

Prior to joining InnovAge, Matilda worked in nursing leadership positions in many senior healthcare settings including; Shea Family Care in La Jolla, Casa De Las Campanas in Rancho Bernardo, San Diego, Villa Healthcare in San Bernardino, Trinity Healthcare Clinic in Palm Springs, AdventHealth in Murieta, and Riverside Medical Clinic in Riverside.

Matilda was born in Cameroon, Africa. She is married and has four adult children. Her family has two German Shepherds, Jethro and Nova. Matilda enjoys gardening and traveling. She speaks French and Pidgin English.

Primary Care Physician

It’s deeply satisfying to work with PACE participants to create a unique and individualized care plan that helps them feel vital, radiant and energetic. That kind of collaboration between the care team and the participant is like a well-orchestrated symphony.

Dr. Thomas earned her Doctor of Medicine and Masters in Public Health at St. George’s University School of Medicine in the West Indies. She also earned a Bachelor of Science, Molecular and Cellular Biology and a Bachelor of Arts, English Literature at the University of Illinois in Urban-Champaign. Dr. Thomas completed a residency in internal medicine at the University of California, Riverside. She is board certified in internal medicine.

With a focus on prevention of chronic conditions associated with lifestyle issues, Dr. Thomas has a passion for helping participants find holistic and comprehensive solutions to maintain their health.

In her spare time, Dr. Thomas enjoys exploring the beauty of California with her husky. She likes to cook and eat delicious food, soak up the sun while sipping coffee and painting with watercolors. She speaks Hindi, Bengali, Spanish and French.

I enjoy working at InnovAge PACE because it allows me to collaborate with participants and the interdisciplinary team members. As a team, we gain different perspectives that allow us to work with participants in a holistic way.

Venice earned a Bachelor Degree in Nursing from California State University in San Bernardino, CA and a Doctorate Degree in Nursing Practice, Adult Gerontology from Loma Linda University in Loma Linda, CA. Prior to joining InnovAge, she worked as a hematology and oncology nurse at City of Hope and a medical and surgical oncology nurse at Eisenhower Medical Center. Her professional interests include primary care for older adults.

Venice grew up in the San Bernardino area and she is a graduate of Rialto High School, Cal State San Bernardino and Loma Linda University. She enjoys serving the community where she has always lived. Venice finds it rewarding to contribute to the wellbeing and health of the residents of the Inland Empire.
