Assistant Medical Director

I believe in empowering my patients to live life to the fullest. The PACE program helps me achieve this goal.

Dr. Sorathia is board certified in family medicine, geriatric medicine, hospice and palliative care. She earned her medical degree from Dow Medical College in Pakistan and completed her residency at Sacred Heart Hospital, Temple University in Philadelphia. Dr. Sorathia also completed a fellowship in geriatrics at the University of Colorado.

Dr. Sorathia strives to help older adults enjoy their lives and accomplishments. She is passionate about the interdisciplinary team approach and all that the PACE program has to offer.

Dr. Sorathia and her husband enjoy skiing and traveling internationally with their children. They especially enjoy visiting national parks.

Regional Medical Officer – Central Region

I saw my mother care for her mother for more than 15 years after a traumatic brain injury, and I watched my father care for his mother as she declined from dementia. They were clearly labors of love – and Herculean labors in their own right. Spending my career at a place that tries to help seniors and families get the help they need is a tremendous honor. 

Dr. Mark Learned most recently served as vice president of Colorado Permanente Medical Group. He earned his medical degree at the University of Washington School of Medicine and his medical residency at the University of Colorado School of Medicine. 

Mark has practiced as a primary care internal medicine physician for over eight years. He has experience as a medical director for Medicaid and charitable programs, with extensive experience in primary care, urgent care, behavioral health, and broad-ranging experiences in leadership at multiple levels of primary care operations, quality, and government relations.

Primary Care Doctor

I strive to give each participant the care and respect they serve and treat them as I would want to be treated. I approach each participant as a whole person, not just their medical situation.

Dr. Woody earned her medical degree from Rocky Vista University College of Osteopathic Medicine in Parker, CO. She completed an internal medicine residency with the HCA healthcare system in Idaho and Colorado.

Her professional interests include geriatric medicine and evidence-based medicine. Prior to entering medical school, she worked for several years as a surgical certified nursing assistant (CNA) at Penrose Hospital in Colorado Springs, CO.

Dr. Woody was born in Colorado but spent her childhood in Ohio. She moved back to Colorado to attend college and medical school. She enjoys spending time with her family. Dr. Woody has a young son and is very close to her brothers and mother. She enjoys cooking, arts and culture – especially the symphony.

Nurse Practitioner

I especially enjoy working with seniors because their wisdom, intuition, and perspective makes them a delight to work with. I strive to take the time to listen and understand each participant, explain complex diagnoses and treatments, and then partner with them in shared decision making. Every participant and family is unique.

Jeff earned his bachelor and master of science degrees in nursing from Columbia University in New York. He is board certified in adult and geriatric primary care. Prior to joining InnovAge, he worked at Internal Medicine Southwest in Littleton, CO. He has more than 10 years experience working in primary care with complicated veteran and geriatric patients.
 
Although Jeff spent his middle and high school years in Durango, CO, he has lived in locations across the globe including Hong Kong and Panyu, China; Sydney, Australia; and Guatemala. He has lived in many cities across the United States including Los Alamos, Chicago, Tuscon, Shreveport, the Bronx and Santa Clara.

Jeff is married and has two teenagers and two dogs. He enjoys watching his children participate in sports and music.

What make me get out of bed every morning are the opportunities to really make a difference and make an impact on other people’s lives.

Dr. Nguyen earned a Bachelor of Electrical Engineering and his Doctor of Dental Surgery from the University of Colorado. Prior to joining InnovAge, he practiced dentistry at private offices in the Denver metro and surrounding areas. Dr. Nguyen is fluent in Vietnamese and was a volunteer teaching the language. He is married with two kids living in Denver for almost 20 years and loves football, soccer, basketball tennis, biking, outdoor activities and is an avid Broncos and Nuggets fan.

Primary Care Doctor

I believe in creating a partnership with my participants.

Dr. Kimball is a board certified wound management specialist physician with additional qualifications in undersea and hyperbaric medicine, vascular medicine, geriatrics, and surgery. She has a passionate focus on being a key opinion leader in the wound care industry and introducing innovative human designs that seek to improve the quality and delivery of care to patients.
 
Dr. Kimball received her medical degree from the University of Texas Medical School, completed her general surgery residency at the University of Colorado Anschutz Medical Campus, and completed vascular surgery research at the University of Utah. Dr. Kimball's primary professional goal is to build a thriving, self-sustainable wound management practice.
 
Dr. Kimball has two children, a son and a daughter.

Primary Care Doctor

I like to team up with my patient, making sure he or she and the family understand why we are doing what we are doing. Working at InnovAge enables me to take the time to communicate and review the treatment plan with patients.

Dr. Kane likes working at InnovAge PACE because she can see her patients and follow them as their care needs and living situations progress—not the norm in primary care practice these days. She brings extensive experience, including three years as Chief of Internal Medicine with the Kaiser Permanente East Clinic in Denver and nine years in nursing home outpatient and inpatient medicine. Dr. Kane earned her Doctor of Medicine (MD) at University of Colorado Medical School and completed her residency in Internal Medicine and board certification in 1988. She is certified in Basic Life Support by the National Advanced Cardiac Life Support Testing Center. She considers her greatest professional success to be working as part of an integrated primary care team, one of the hallmarks of InnovAge PACE. She is married with twin daughters and a dog, and works just three days a week so she can care for her elderly parents. She enjoys reading, drawing and skiing.
