Center Director

I love working with older adults. It has been my passion since beginning as a certified nursing assistant when I was 17. My personal passion about working with this population and keeping them as independent as possible is a perfect match for InnovAge. I love the services we provide our participants.

Roslyn Paulson began her career with InnovAge as center director at the Thornton PACE Center before transitioning to her current role as director of the Lakewood PACE Center in 2017. Roslyn began her career in healthcare at the age of 17 as a certified nursing assistant. She has since worked in long-term care and senior nursing facilities, in roles ranging from social work and admissions to marketing and case management. She is also a licensed nursing home administrator. Roslyn earned her bachelor's degree in social work from Colorado State University.

Regional Medical Officer – Central Region

I saw my mother care for her mother for more than 15 years after a traumatic brain injury, and I watched my father care for his mother as she declined from dementia. They were clearly labors of love – and Herculean labors in their own right. Spending my career at a place that tries to help seniors and families get the help they need is a tremendous honor. 

Dr. Mark Learned most recently served as vice president of Colorado Permanente Medical Group. He earned his medical degree at the University of Washington School of Medicine and his medical residency at the University of Colorado School of Medicine. 

Mark has practiced as a primary care internal medicine physician for over eight years. He has experience as a medical director for Medicaid and charitable programs, with extensive experience in primary care, urgent care, behavioral health, and broad-ranging experiences in leadership at multiple levels of primary care operations, quality, and government relations.

Nurse Practitioner

Personalized healthcare and positive social interaction are vital to maintaining strong individuals and a strong community. PACE makes this possible for seniors.

Amy earned a master’s degree in nursing and healthcare practice from the University of Iowa and her post-master’s family nurse practitioner certificate from Regis University in Denver, CO. She also earned a Bachelor of Arts Degree from DePaul University in Chicago, IL. Prior to joining InnovAge, Amy worked as a nurse practitioner for five years and was an intensive care unit nurse for nine years.
 
Amy is from Chicago, IL. Her husband is a teacher and they have a daughter in elementary school. In her spare time, Amy enjoys doing embroidery and playing board games. She loves to take family trips to arcades and says she is very good at winning stuffed animals!

Nurse Practitioner

My goal is to help participants enjoy the best quality of life. I believe listening and collaboration are essential to meet the healthcare goals of each individual.

Angela earned her Bachelor of Science Degree in Nursing from Middle Tennessee State University and her Master of Science Degree in Nursing from University of Colorado. Her master’s degree was focused on adult gerontology primary care. Angela is board certified as an Adult Gerontology Nurse Practitioner from the American Association of Nurse Practitioners.
 
Prior to joining InnovAge, Angela worked with a primary care practice that provides care in patients’ homes. She also worked in hospitals in intensive care and neurology/trauma. Angela is a member of the Colorado Nurses Association, Gerontological Advance Practice Nurses Association, American Association of Nurse Practitioners and the American Nurses Association.  
 
Angela is from Tennessee and moved to Colorado in 2012. She and her husband enjoy spending time with their two cats, Marty and Ruthie. They also enjoy outdoor activities like hiking and snowboarding.

Nurse Practitioner

I love that the PACE model promotes participant independence and that I can contribute to their quality of life by coordinating care with them, their families, and their entire team.

Elizabeth completed her doctor of nursing practice in adult-gerontological primary care nurse practitioner degree at East Carolina University in Greenville, North Carolina. She is board certified as an ddult-gerontological nurse practitioner by the American Association of Nurse Practitioners. She attended the University of Iowa where she earned a master’s degree in nursing and a bachelor’s degree in health promotion.
 
Prior to joining InnovAge, she worked as a nurse practitioner in the home, nursing facilities, and hospice. She has experience in regenerative medicine and joint injections. Elizabeth also worked in hospital neurology and cardiology units.
 
In her spare time, she likes running, yoga, and indoor cycling. She and her husband live in Denver and have two rescue dogs. Elizabeth enjoys camping, going to sporting events and concerts. She grew up in Iowa and moved to North Carolina for graduate school.

Area North Medical Director - Colorado

PACE is a wonderful model that allows me to do what I love – care for older adults with complex needs by working as part of a tight-knit team of skilled professionals.

Dr. Silversmith earned his medical degree at the University of Pittsburgh School of Medicine, a Master of Science degree in Behavioral Decision Research at Carnegie Mellon University, and a Bachelor of Arts degree in Cognitive Science/Psychology at Yale University. He completed a residency in internal medicine at Montefiore Medical Center in NY. Dr. Silversmith is board certified in internal medicine.
 
A passion for caring for older adults is a theme that runs through all of Dr. Silversmith’s work experience. Prior to joining InnovAge PACE, he worked at Mount Sinai Medical Center in New York City where he helped develop the Mobile Acute Care Team, which treats acute conditions in the home instead of the hospital. He staffed and helped advance a “high risk clinic” that provides primary care, care coordination, and social services to people who are treated frequently in hospitals and emergency departments. He also worked at Kaiser Permanente as a home visit doctor for home-bound patients.

Nurse Practitioner

One must listen to heal.

During her college years in Washington D.C., Patricia Hughes spent summers waitressing in Estes Park, Colo. She was hooked and moved to Colorado permanently. She earned her Master’s degree from the University of Colorado Health Sciences Center and her doctorate in nursing practice from Regis University. She specializes in adult care. Patricia has shared her expertise by serving as a preceptor for nurse practitioner students at the Denver Veterans Affairs Medical Center (VAMC). Patricia developed a pamphlet on the pros and cons of the prostate cancer-screening test, which was distributed to eastern Colorado VAMC clinics. Patricia was born and raised in New Jersey and has two sons and two granddaughters. She enjoys going for long walks, watching movies, reading and listening to jazz. She is also in the process of teaching herself Spanish.

Primary Care Doctor

Great primary care is about creating a partnership between a patient and their care team. InnovAge puts the patient’s goals and individualism first.

Dr. Johnson is board certified in internal medicine. He earned his medical degree at Villanova University and Drexel University College of Medicine in Philadelphia, PA. He completed his residency training at University of Colorado with his primary clinic at Denver Health Westside. Dr. Johnson served an additional year as the chief resident in quality and patient safety.

In addition to working for InnovAge, Dr. Johnson is the founder and medical director for the DAWN Clinic (Dedicated to Aurora’s Wellness and Needs). He also serves as the director of community health for Senator Rhonda Fields of Aurora.

Dr. Johnson loves to ski, hike, play games, watch the Broncos and listen to live music.

I believe in creating a partnership with my participants.

Dr. Kimball is a board certified wound management specialist physician with additional qualifications in undersea and hyperbaric medicine, vascular medicine, geriatrics, and surgery. She has a passionate focus on being a key opinion leader in the wound care industry and introducing innovative human designs that seek to improve the quality and delivery of care to patients.
 
Dr. Kimball received her medical degree from the University of Texas Medical School, completed her general surgery residency at the University of Colorado Anschutz Medical Campus, and completed vascular surgery research at the University of Utah. Dr. Kimball's primary professional goal is to build a thriving, self-sustainable wound management practice.
 
Dr. Kimball has two children, a son and a daughter.

Professional Bio

I enjoy talking with my patients and finding out about their lives and what they did for a living. They tell me some very interesting things!

Dr. Bottenbley has a way of making patients relax when providing dental care at the PACE center. He attended Harvard School of Dental Medicine, where he received his Doctor of Dental Medicine (DMD), and completed his Oral Surgery internship at the University Hospital in Aachen, Germany where he had to do “business as usual” in the German language. He completed his residency in General Practice at the Dept. of Dentistry, Montefiore Medical Center in the Bronx, New York, and another residency in Oral an ad Maxillofacial Surgery at Bellevue Hospital, New York University training program. He taught oral surgery at the University of Colorado School of Dental Medicine, and held a fellowship at New York University College of Dentistry. He enjoys fly fishing and hunting waterfowl and upland game. His greatest accomplishments are his “successful marriage to my wonderful wife, and a son I am very proud of.”

Life is filled with challenges and triumphs. Our perspective makes the difference; I choose joy. I hope to impact others with that joy by serving those who are in the most need.

Raquia Denson earned her bachelor’s degree in biology from Tuskegee University in Alabama, and her doctor of dental surgery degree from the University of Iowa. Prior to joining InnovAge, Raquia worked as dentist-in-charge for a community health center in Dallas. She also has experience treating older adults in nursing homes, rehab facilities, and assisted living communities. She has treated patients in the department of corrections and has owned her own private practice. Her passion is treating the underserved. Raquia was born in Gary, Indiana and has lived in Georgia, Alabama, Illinois, Iowa, Texas, Minnesota, and Colorado. She is a proud aunt to three nieces and three nephews. She has a Pomeranian-husky mix named Roxie. In her spare time, Raquia likes to craft, travel, dance, volunteer, and cheer for her beloved Chicago Bears!

Primary Care Doctor

I enjoy working with older adults because I have the privilege of listening to their life stories. I hear their perspectives on the historical events they’ve lived through.

Dr. Leiding is board certified in Geriatric Medicine and Palliative Care. He earned his Doctor of Medicine (MD) degree from Loyola University in Chicago, held a fellowship in geriatrics at Rush University Medical Center and completed his residency in internal medicine at Caritas St. Elizabeth’s Medical Center in Boston. His specialties are dementia, maintaining function and quality of life, and end-of-life decision making. He enjoys working at InnovAge because the interdisciplinary team is quickly able to bring together a variety of services under one roof that participants need to lead the fullest lives possible. Dr. Leiding is married, has a dog, and enjoys exploring the outdoor activities that Colorado has to offer.

Primary Care Doctor

I take pleasure in working at PACE and establishing long-term relationships with participants. I have the satisfaction of seeing their appreciation for life, learning from their wisdom and listening to the best stories of days gone by.

A central value in PACE care is supporting participants so they can maintain their independence for as long as possible. As a physician practicing for 17 years, this philosophy is something Dr. Pham believes in, because she has seen the difference the PACE program has made in the lives of those she serves. Dr. Pham is board certified in Internal Medicine and has an interest in Geriatrics. She received her Doctor of Medicine (MD) from the University of Iowa College of Medicine in Iowa City, and completed her internship and residency at Penn State Milton S. Hershey Medical Center in Hershey, Pennsylvania. Dr. Pham grew up in Vietnam, came to the U.S. as a teenager and lived with a foster family in Iowa. Married now and a mother of twins, she is learning to play guitar and sings in her church choir.
