Welcome to Sacramento PACE

Our passion and mission at InnovAge is to provide services to the most vulnerable seniors to help them age in their own homes with dignity. Right now, frail seniors and their families need us more than ever. We are excited to collaborate with Adventist Health and Eskaton to bring PACE services to seniors in the Sacramento area. We look forward to welcoming community members to the center and to learning more about the PACE model of all-inclusive care.”

We’re excited to join InnovAge and Eskaton in this transformational service that gives Sacramento-area seniors more choices for living their lives and more support in managing their health. The comprehensive, coordinated care provided through the PACE center will improve independence and outcomes while relieving stress and reducing healthcare costs. Through this innovative program, we are expanding our mission of living God’s love by inspiring health, wholeness and hope into our Sacramento community.

Congratulations to Maureen and everyone at InnovAge on the virtual opening of their newest center in Sacramento. PACE is undoubtedly the best community based program available to provide healthcare and social services to seniors who would otherwise not be able live at home. Eskaton is proud to collaborate with Adventist Health and InnovAge to bring this new center to the Sacramento region. Our participation in the PACE center is aligned with Eskaton’s nonprofit mission to enhance the lives of seniors through innovative health, housing and social services. InnovAge has earned a stellar reputation, and the innovation and scale their centers bring to communities like Sacramento is desperately needed to provide quality of life for seniors and reduce hospitalization and healthcare costs.
