PACE participants receive most of their healthcare and social support at a PACE center in their community. Each person in the program has a team of medical experts and a personal care plan that is focused on providing a personalized PACE experience. • Does anyone live at an InnovAge PACE center?

No. The goal of PACE is to keep older adults living safely at home. Care teams and participants decide how often the participant attends the PACE center for medical needs and social engagement. Participants return to their own homes each night.

  • Can I join without going to a PACE center?

Your attendance at the PACE center is based on your medical and social needs. One of the benefits of PACE is the personalized care each participant receives. Some participants only visit the center for medical appointments. Others come more often to socialize, eat meals and enjoy activities.

  • Can I keep my regular doctor?

PACE participants see a doctor and have access to a full medical team at their local PACE center. The doctors and many members of the medical team are experts in senior healthcare and are dedicated to giving personalized care to each participant.

  • Where do I see my primary care doctor?

PACE participants see their primary care doctor at the PACE center.

  • I really like my specialist (cardiologist, oncologist, etc.). Can I keep seeing him/her after I join PACE?

InnovAge has a network of specialists in each community. If your specialist is already in the InnovAge network, and the care is coordinated through PACE, you may be able to see the same specialist. If the care is approved by your medical team but your specialist is not in the InnovAge network, InnovAge will coordinate your care with an in-network specialist.

Our PACE clinics arrange all specialty medical appointments and transportation for participants. If a family member or friend is unable to go with you, a member of your care team can accompany you.
