Join the InnovAge Provider Network

InnovAge is a Program of All-Inclusive Care for the Elderly (PACE) organization that helps older adults remain independent in their homes and communities for as long as safely possible. Through coordinated medical care, social support, transportation, and other covered services, InnovAge works with participants, caregivers, and provider partners to support each participant’s health and daily needs.

External affiliate providers interested in joining the InnovAge network should submit a request email that includes the organization’s legal name, DBA name if applicable, full-service address including ZIP code, and the type(s) of services provided. Each request will be reviewed, and a response will be provided within 10 business days.
