Outside providers of InnovAge's Program of All-inclusive Care program (PACE), known as LIFE in some communities like Philadelphia, are valued partners in providing the personalized, coordinated care to each individual enrolled in the program. Here, you can find resources and other information.

Provider Manuals
Manuals outline how contracted providers can best work with InnovAge's PACE model of care and the various disciplines and departments throughout our organization. An InnovAge Network Provider Manual is available for contracted providers in each state in which we operate.

Claims
In addition to coordinating and providing care for individuals enrolled in InnovAge PACE, we also process and pay health benefits for the individuals enrolled in the program.

EpicCare Link
As InnovAge implements Epic EMR across the enterprise, outside clinical providers will be able to connect with InnovAge and access the health history and more resources for patients (participants) who have been referred to them.

HHS Secretary Kennedy Visits InnovAge & New HHS Study Highlights Benefits of PACE Model

InnovAge recently welcomed U.S. Health and Human Services Secretary Robert F. Kennedy Jr. to its Thornton PACE center, where he met with participants and care team members and observed firsthand how integrated care supports older adults with complex health needs.
