InnovAge network providers are an extension of PACE interdisciplinary teams (IDT). IDT members work together to ensure participants receive PACE services needed to remain healthy and safe in their communities.

To assist our valued providers, we provide a detailed manual on how best to work with the InnovAge PACE model of care and various disciplines and departments throughout our organization. 

Please review and use this manual with leadership, business offices, and internal staff. Our common goal is to deliver the absolute best outcomes for participants in our care.
