The St. Bart’s LIFE center opened in 2018 and is located within the footprint of the former St. Bartholomew Catholic School in Northeast Philadelphia. It is next door to a 100-year-old church and parish. Participants enjoy Zumba classes, horticulture, live entertainment, group outings, and spiritual support groups.

The St. Bart's LIFE center is one of InnovAge's centers for seniors in Philadelphia, Pennsylvania. Other centers are located in the communities of Pennypack, St. Bart's, and at Henry Ave.

LIFE is designed to keep seniors independent and at home for as long as possible.

Senior care decisions are never easy, especially when dealing with complex health issues that require multiple services. InnovAge LIFE, known nationally as the Program of All-inclusive Care for the Elderly (PACE), can help by providing healthcare and social services to seniors in Philadelphia - without having them move into a nursing facility. InnovAge operates multipleLIFE centers for older adults in Philadelphia. These are located in the communities of Allegheny, Pennypack, St. Bart’s, and at Henry. Ave.

Each participant works with a team of medical experts to develop a care plan that is personalized to meet their evolving medical and social needs. Senior care is not 'one sized fits all,' which is why each participant has a customized LIFE experience to meet their individual needs.

Each InnovAge LIFE center includes:

  • Spaces for medical care, physical therapy, behavioral health, and more
  • Day rooms and dining spaces for socialization among our participants
  • Population-specific design elements, such as grab bars and secured entries and patios
