Connected healthcare is the core of PACE.

PACE services are provided by connected care teams called interdisciplinary teams (IDT), which are responsible for coordinating 24-hour care. The PACE benefit provides all Medicare and Medicaid services as well as additional services not covered by Medicare and Medicaid, as authorized by the IDT. PACE provides coverage for prescription drugs, doctor care, transportation, home care, checkups, hospital visits, and even nursing home stays whenever necessary. Care is coordinated at an InnovAge PACE center near your home. Here are some of the services covered by PACE:

• Emergency care
• End-of-life care
• In-home assistance
• Medication management
• Nutrition
• Physical, occupational and speech therapy
• Respite care
• Social engagement
• Specialty care
• Transportation

Preventive, primary care, and behavioral health services are provided at the PACE center, as well as dental and vision care, nutritional meals, recreational therapy, and social engagement.  Also included is hospitalization, post-hospital care, laboratory/X-ray services, and durable medical equipment.

InnovAge’s team of healthcare professionals is focused on maintaining and improving your health – in partnership with you and your loved ones. The team has experience caring for older individuals and they want to know what is most important to you. They get to know you, your living situation, your preferences, and your goals.

Along with the results of your initial and periodic health assessments, the team uses this information to develop and update your plan of care.

Each participant in the Program of All-inclusive Care for the Elderly (PACE) has a customized care plan designed just for them.

Please note that InnovAge PACE participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from InnovAge or a person or company authorized by InnovAge. InnovAge PACE participants may be fully and personally liable for the costs of any unauthorized or out-of-network services.
