Medication Management

Medication management is included your personalized PACE care plan.

Our team makes sure you’re taking the right medications and the right doses, all with the goal of preventing medication errors.

Prescription drugs and prescribed over-the-counter medications are included in PACE, at no cost. You will receive your medication(s) at the PACE center or we will deliver them to your home.

Please note that InnovAge PACE participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from InnovAge or a person or company authorized by InnovAge. InnovAge PACE participants may be fully and personally liable for the costs of any out-of-network services.
