A Simple way to securely share information for better patient care

What is EpicCare Link?
EpicCare Link is InnovAge's web-based portal where outside clinical providers can have secure, real-time read-only access to the health records of InnovAge participants who have been referred to them. Outside providers can also connect with InnovAge's interdisciplinary care team. 

What is available in EpicCare Link?
Information available within EpicCare Link includes progress notes, lab results, discharge instructions, consult notes, and more. You can also communicate with InnovAge providers.

How do I access EpicCareLink?
InnovAge is rolling out Epic in phases across the organization. As of fall 2022, InnovAge's Richmond and Peninsula centers in Virginia have transitioned to the new EMR. Outside providers of participants for these two centers can request access via the link below. As the EMR is implemented at additional locations, contracted providers will be contacted by InnovAge with more information. 

InnovAge requires that each account be associated to an individual’s organization email. General email address (frontdesk@company.com) may not be used for account creations. Non-Business/Non-Company email services (e.g., Gmail, Yahoo, Hotmail, etc.) are not permitted.

Medication management is included your personalized PACE care plan.

Our team makes sure you’re taking the right medications and the right doses, all with the goal of preventing medication errors.

Prescription drugs and prescribed over-the-counter medications are included in PACE, at no cost. You will receive your medication(s) at the PACE center or we will deliver them to your home.

Please note that InnovAge PACE participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from InnovAge or a person or company authorized by InnovAge. InnovAge PACE participants may be fully and personally liable for the costs of any out-of-network services.
