Transportation

InnovAge provides transportation from your home to the PACE center and to off-site medical appointments.

From the moment participants are picked up at home to the moment they are escorted into an InnovAge PACE center, they are surrounded by people who respect and encourage them to make the most of their PACE experience.

InnovAge drivers are highly trained and are important members of your care team. Our skilled drivers deliver medications and medical supplies to your home. They get to know you and help make sure you have everything you need for your day at the center or your medical appointment.

InnovAge PACE has an extensive provider network in each community we serve.

Medical specialists in our provider network treat many conditions. PACE participants have access to medically necessary dentures, hearing aids and eyeglasses at no cost. These benefits are not always included in other senior healthcare programs. Other conditions treated in PACE include but are not limited to:

• Cancer
• Dementia, including Alzheimer’s disease
• Depression and other mental health conditions
• Diabetes
• Emphysema
• Heart disease
• Kidney disease

If the care team determines you need treatment from a medical specialist, InnovAge will coordinate appointments and care at no additional cost to participants.

Please note that InnovAge PACE participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from InnovAge or a person or company authorized by InnovAge. InnovAge PACE participants may be fully and personally liable for the costs of any out-of-network services.
