Open Enrollment, PACE & InnovAge: What Seniors and Families Should Know

When it comes to healthcare, choosing the right support can feel complicated. During Medicare Open Enrollment, many seniors and families take time to review their options and ask an important question:

“Is there a better way to support my loved one’s health, independence and quality of life?”

For some older adults, the answer may be PACE (Program of All-Inclusive Care for the Elderly), an integrated care model that brings healthcare and supportive services together to help eligible seniors continue living safely in their own communities.

Here are answers to some common questions about Medicare Open Enrollment, PACE and InnovAge.

What is Medicare Open Enrollment?

Medicare Open Enrollment is an annual opportunity for people with Medicare to review their coverage and make changes if needed. Open Enrollment period runs from October 15 through December 7, with plans taking effect on January 1.

It’s a good time to ask:

  • Are my healthcare needs changing?

  • Am I getting the support I need?

  • Are there services I wish were easier to access?

While Medicare Open Enrollment occurs once a year, PACE enrollment is available year-round for individuals who qualify.

What is PACE?

PACE (Program of All-Inclusive Care for the Elderly) is a Medicare and Medicaid program designed to help eligible older adults receive comprehensive medical and supportive services while continuing to live in their communities.

PACE is designed for adults 55 years and older who need more support than traditional healthcare alone may provide. Instead of managing multiple doctors, appointments and services separately, PACE brings care together through one coordinated team, known as the interdisciplinary team, or IDT.

The team may include:

  • Primary care physician

  • Registered nurse

  • Social worker

  • Physical therapist

  • Occupational therapist

  • Recreational therapist or activity coordinator

  • Dietitian

  • PACE center manager

  • Home care coordinator

  • Personal care attendant

  • Driver

Each participant’s dedicated care team works with them to understand their goals, create a personalized care plan, and coordinate the services they need. Depending on individual needs, services may include:

  • Medication Management

  • Social Engagement

  • In-Home Assistance

  • Specialty Care

  • Transportation

  • Coordinated Healthcare

Every person’s journey is different. That’s why care is built around the individual.

Who is InnovAge?

InnovAge is the largest provider of PACE based on participants served. We are committed to expanding this critical program to serve as many seniors as possible across the country. Headquartered in Denver, Colo., InnovAge offers PACE in Colorado, California, Florida, New Mexico, Pennsylvania and Virginia.

The goal is simple: help seniors live life on their terms – by aging in place, in their own homes and communities, for as long as safely possible.

Who may qualify for PACE?

  • Participants in PACE must meet the following eligibility requirements:

  • Be at least 55

  • Be rated as eligible for nursing facility level of care as determined by the state in which you live

  • Able to live safely at your home and in your community with the support provided by PACE

  • Live in a community served by InnovAge

While most people enrolled in PACE qualify for - or have - Medicare and Medicaid - it is not a requirement.

What should families consider when exploring PACE?

Every family’s needs are different. When exploring PACE, consider how care will be coordinated, whether there is a team that understands your loved one’s needs and goals, and what support is available to make daily life easier, from transportation and activities to help at home.

It’s also important to think about the future. Could this support your loved one’s desire to remain independent? Will PACE help your family navigate healthcare decisions along the way?

PACE brings healthcare and support services together to help eligible older adults live safely, independently, and connected to their community. If you’re wondering whether PACE may be right for you or a loved one, InnovAge can help you understand eligibility, available services, and next steps.
