September is Falls Prevention Awareness Month: Moving from Fall Response to Fall Prevention

At InnovAge, we believe preventing falls means protecting what matters most: independence, confidence, mobility and quality of life. Falls are not an inevitable part of aging. With early identification, personalized support and proactive care, many falls can be prevented so older adults can continue doing the things they enjoy and stay connected to their communities. During Falls Prevention Awareness Month this September, InnovAge is highlighting our commitment to a proactive approach to help keep our participants safe.
 

A proactive approach to keeping older adults safe

Traditionally, fall prevention efforts have often focused on what happens after a fall occurs. At InnovAge, we are working to identify risks earlier and take action before a fall happens. Through programs and practices such as AgeWell, enhanced fall-risk screening, participant education, therapy expertise and interdisciplinary collaboration, our teams work together to understand each participant’s unique needs and create personalized strategies to support safety and independence. The impact of this proactive approach is reflected in our outcomes: InnovAge participants experience 28% fewer falls on average compared with the National PACE Association’s estimated average. This achievement reinforces the value of identifying risks early, coordinating care across disciplines and providing personalized support before a fall occurs.

Our approach includes:

  • Identify: Using standardized tools and data to better understand individual risks
  • Act: Implementing personalized interventions, education, and support
  • Monitor: Tracking progress and adjusting care as needs change
  • Prevent: Reducing risk while helping participants remain active and engaged

Sharing Innovation with the PACE Community

InnovAge therapy leaders will share our fall prevention journey at the National PACE Association (NPA) Quality Symposium in Denver this October, highlighting how data, clinical expertise, and interdisciplinary collaboration are helping transform the way we approach falls. This work reflects the strength of the PACE model: bringing together a team of healthcare professionals to address the whole person, identify needs early and provide the right support at the right time. Fall prevention is about more than reducing injuries. It is about helping older adults maintain the confidence to move safely, participate in meaningful activities and continue living life on their terms. Because prevention isn’t just about avoiding falls — it’s about preserving independence and quality of life.
