InnovAge Northern Colorado

InnovAge offers all-inclusive care to seniors in Colorado. PACE centers are located in Aurora, Denver, Lakewood, Northern Colorado, Pueblo, and Thornton.

PACE is designed to keep seniors independent and at home for as long as possible.

Senior care decisions are never easy, especially when dealing with complex health issues that require multiple services. InnovAge PACE – Program of All-inclusive Care for the Elderly – can help by providing healthcare and social services to seniors – without having them move into a nursing facility. InnovAge operates six PACE centers for older adults in Colorado. These are located in Aurora, Denver, Lakewood, Northern Colorado, Pueblo, and Thornton.

Each participant works with a team of medical experts to develop a care plan that is personalized and regularly updated to meet their evolving medical and social needs. Senior care is not ‘one size fits all,’ which is why each participant has a customized PACE experience to meet their individual needs. 
 

Each InnovAge PACE center includes:

  • Spaces for medical care, physical therapy, behavioral health, and more
  • Day rooms and dining spaces for socialization among our participants
  • Population-specific design elements, such as grab bars and secured entries and patios
In Colorado, in addition to PACE, InnovAge owns two senior living facilities, managed by ComCap Management. InnovAge's two senior living facilities are not affiliated with InnovAge PACE.

InnovAge Colorado PACE - North

The Northern Colorado PACE center opened in 2015 and serves seniors in Fort Collins, Greeley, Loveland, and other surrounding communities. The building features a mountain-resort style entryway with wooden beams, brick, stone, and stucco. Historically-relevant local artwork is displayed along the curved hallway that provides access to all areas of the center and serves as a walking loop for participants (15 laps equals one mile). Some favorite participant activities include bingo and chair tap dancing.

The Northern PACE center is one of six InnovAge PACE centers in Colorado. Other centers are located in the communities of Aurora, Denver, Lakewood, Pueblo, and Thornton.
